Participant Consent Form

This consent form informs you about how your personal data will be used during your participation in the Level Up Digital project. Your consent is voluntary, and you have the right to refuse or withdraw at any time without penalty.

During your participation in the Level Up Digital program, we will collect:

• Feedback about your experience in the program

May we use photos or videos?

During the program, we may take photos or videos for documentation and promotional purposes. We will ONLY use images if you give your explicit consent below.

Your rights regarding photos:
